Please bring your dental insurance card to your first appointment, and arrive early to complete your introductory paperwork. If you are relocating to the area, please request a copy of your dental records from your previous dentist.

Please bring a list of any medications you’re currently taking.

Many patients are unaware of the details of their dental insurance coverage. Your dental benefits are determined by the agreement reached between your employer and your insurance company. Please check your employer's insurance benefits booklet to better understand the benefits provided as part of your insurance coverage. 

Depending on your employer's coverage, the portion of the patient payment covered for treatments varies. Based on the information submitted to us, an estimate of the amount covered by your insurance company will be provided at the time of your treatment. 

As a courtesy to our patients, we’ll file all insurance claims. If the amount paid by the insurance carrier is less or more than the estimate, you’ll be invoiced for the difference or given credit on your account.
